Download that file:

Find the same filename on your Win10 computer. Replace the contents with your downloaded updated file.

Thanks @emyoulation , I have done this on my Linux computer. I think @Martnal was needing the same info for his Windows setup. I updated GRAMPS to 5.2 which includes the new form.

Emyoulation, I tried that and it didn’t change anything, then I remembered I had to add the Form as a Source. Thank you! All fixed.
